School Based Health Center
The School Based Health Center (SBHC) offers medical, dental, and social work services to all Silver Lane students. The nurse practitioner is available for well child care/school physicals, sick visits, immunizations, and health education programs. The dental hygienist provides oral health screenings, dental cleanings, sealants, oral health education, and referral services. Social work services include individual, group, and family counseling, as well as resource and referral whenever needed.
All SBHC staff work collaboratively with parents, school personnel, and community agencies in addressing the various needs of the students and families at Silver Lane. We recognize that many children have an identified primary care provider. The SBHC serves as a supplement to, rather than a replacement for, this health care relationship. Children who do not have a primary care provider or health insurance receive care as well as assistance in accessing appropriate support services in the community.
The SBHC follows the school calendar year and is open Monday through Friday, 8:30am to 3:30pm. Parent permission is required before any student can receive services. For further information or to speak with individual staff members call 860-622-5514.
