Student Registration Checklist

 

After submitting your child’s Pre-Registration Form, you will receive an email. Please follow the directions in the email to create a Parent Account on PowerSchool and link a student or add a student to your existing Parent Account. Sign-In to your PowerSchool Parent Account. Complete all FORMS--ENROLLMENT, then make an appointment to bring the following to Central Registration:

  • Parent/Guardian’s Photo ID
  • Proof of student age (birth certificate, baptism, passport)
  • Proof of Residency (link opens in new page)
    • If you are a guest in someone else’s home (not on the lease/mortgage), you must provide a Certificate of Residency form each year.
  • Guardianship documents (if applicable)
  • Individualized Education Program (IEP)/504 Plan, if applicable
  • Transcript/Report Card from previous school, if available
  • Recent physical exam and record of immunizations (necessary before the first day of school)
    • Students entering kindergarten must be 5 years old on/before September 1st of the kindergarten year, and need a physical examination no more than 12 months before the first day of school. 
    • Students entering 7th or 10th grade need a physical examination no more than 18 months before the first day of school. 
    • Any student new to the State of Connecticut will need a current physical examination. 
    • If you do not have medical records, please contact Central Registration so we may help you obtain the records, immunizations, and/or physical examination.
